Max Vantage Replacement Card Submission Form

Once you have submitted your request, you will receive a replacement card via US Mail within 7-10 business days. Your account history and current point total will be transferred to a new card and account number, making your old card inactive. If you dine with us while waiting for your replacement, you can still earn points by having your account looked up using your telephone number at any Max Restaurant.
